To ensure we maximize your treatment experience, please complete both the Client Intake and Photography Release forms prior to your first appointment. If forms are not completed in advance, some of your scheduled treatment time may need to be allocated for paperwork.

For clients under the age of 18, a parent or legal guardian must review and submit the Parental/Guardian Consent form to successfully book an appointment to receive professional facial services.
